Does a resident have to provide documentation of their medical conditions and medications?

Yes, a resident is required to provide information regarding their medical conditions and current medications and/or other treatments that they may be receiving. For example Diabetes requiring insulin, as detailed in the application form.

Evidence of assessment by the Aged Care Assessment Team is required.   Personal Care staff only have access to information regarding a residents medical condition. Such information is treated as confidential.
